About your first visit
On your first visit, please arrive 30 minutes prior to your appointment time to allow time to fill in registration information.
We see new patients, ages 14 years and older.
Also, please bring the following items with you:
-Valid Drivers license or State ID.
-The bottles of all the medications you take on a regular basis, including over-the counter medications and vitamins.
-List of surgeries or medical procedures you have had.
-Payment for your co-pay or any non-covered services. We accept cash, check, Visa, Mastercard, Discover and American Express.

Dr. Williams is currently contracted with:
Florida Blue, Blue Cross and Blue Shield
Empire Plan - BC&BS only (not United Health Care version)
Florida Hospital Health Network including
Beach Street, PHCS
Florida Health Care Plans- all products
Volusia Health Network
Medicare and all supplement plans
AARP Medicare Supplement
United Health Care Supplement Plan to Medicare
The only HMO plans we are contracted with are Florida Health Care Plans and Florida Blue HMO
Unfortunately, we will no longer be accepting United Health Care Plans commercial insurance. We will still accept it as a secondary to Medicare B. Existing patients with United Health Care should have received a letter. Beginning in June patients covered by United Health Care can choose to stay with us as self-pay patients. Patients are encouraged to call UHC and express their concerns about losing Town Center Family Care as a network provider.
We do not have a contract with Medicaid. We cannot take Medicaid patients at this time. Dr.Williams attends the Halifax Family Medicine Residency clinic as teachers and that clinic is contracted with Medicaid which can explain why they are sometimes listed as providers.
Self Pay - we are happy to take cash only patients. Payment is due in full at the time of service.
Same day appointments are available for established patients - please call early in the morning and we will fit you into the schedule the best we can.
Credit cards and personal checks are accepted.
Co-Pays and Past Due Balances
We will collect all co-payments and past due balances at the time of service. If you are unable to make payment at the time of service, we will reschedule your appointment. We use an independent billing company to handle our collections
No Shows/Late Cancellations
There will be a $25 charge for those who skip their appointments or cancel with less than 24 hours notice. New patients who no show will have to settle their balance prior to rescheduling their new patient appointment.
If you need a prescription refill, please call your pharmacy and they will fax or email us the request. If you desire a new prescription or to change your pharmacy, then please call our office during regular office hours so that we will have access to your chart to ensure the best course of treatment. Dr. Williams does not routinely prescribe narcotic pain medications for chronic non-malignant pain. We are unable to prescribe or renew medications for new patients prior to the first appointment due to state law.
